For old scars, I recommend exfoliation. You can manually exfoliate: scrubs, ';Buf-puf'; sponges, etc. tend to work well. If you use a scrub once a day, this should help reduce the scars.
Also, you can chemically exfoliate. Using a lotion that contains alpha- and beta-hydroxy acids is generally the best way to do this. I use Eucerin Intensive Care lotion, at night; it both moisturizes and exfoliates.
For current blemishes, it's best to choose a regimen and stick to it. I use Duac during the day, which is a blend of Benzoyl Peroxide and Clindamyacin. It's prescription-only, though, so if you can't talk to a doctor about it, try using an over-the-counter cream that contains Benzoyl Peroxide. Benzoyl Peroxide tends to be drying, so it's best to use a lotion along with it. It works by fighting the infections in your pores.
At night, I use a Retinol cream called Tazorac. It's also prescription-only, so if you don't have access to a dermatologist, try using a salicylic acid gel instead. Either of these will help your skin renew itself more quickly, and work to dissolve dirt in your pores.
There are over-the-counter and prescription scar creams, but they tend to be expensive, and I have no experience with them. If you have any major scars, though, they might be worth investigating.
Be sure to wash your face thoroughly with a gentle cleanser (such as Cetaphil) each morning and evening. And since acne medications can increase the skin's sensitivity to the sun, use sunscreen (or a moisturizer with SPF) during the day.
